Service Description

Mental Health Evaluation A Mental Health Diagnostic Evaluation is a comprehensive assessment designed to better understand an individual’s emotional, psychological, and behavioral health. This process helps identify underlying concerns and guides next steps for care. What to Expect: Clinical Interview: A detailed conversation about your symptoms, personal history, medical background, and lifestyle factors. Screening Tools & Assessments: Standardized questionnaires or checklists to evaluate specific areas of mental health. Observation: Professional insights into behavior, mood, and communication during the evaluation. Diagnosis: Identification of mental health conditions using established criteria (such as the DSM-5). Recommendations: A personalized treatment plan that may include therapy, referrals, or other supportive resources. (Note: Therapists cannot prescribe medication but can provide referrals for medication management if needed.)


Cancellation Policy

Please cancel within 24 hours. You will be assessed a $25 fee for not cancelling and or full session price if within 2 hours of session. We understand life happens so please call your therapist if you need to cancel less than 24 hours. If you are late for a session (9 minutes allotted) please note that time is still counted towards your allotted time, as each client deserves to have their therapist on time. If you are a no call no show and/or our office does not hear from you, you will be charged for your full session. Balance must be paid prior to rescheduling a new session. If you reschedule you must confirm the reschedule date and time. All group fees, trainings and The Jar Coffee & Convo are non refundable. Supervision and Preceptorship Fee are non refundable once service has started. If you should cancel prior to- a $250 administration fee will be subtracted from payment. Please understand this is an independently ran private practice. We thank you for understanding this practice policy and guidelines.
